Trazodone is a serotonin antagonist and reuptake inhibitor (SARI) that was originally approved by the FDA in 1981 for the treatment of major depressive disorder. Today, trazodone is prescribed almost exclusively at low doses as a sleep aid rather than as an antidepressant, making it one of the most commonly prescribed medications for insomnia in the United States. Mechanism of action

The compound is metabolized primarily by CYP3A4 to its active metabolite meta-chlorophenylpiperazine (mCPP), which has serotonin agonist properties. The dose-dependent shift in pharmacological profile explains why trazodone is sedating at low doses but can be activating or cause more typical serotonergic side effects at antidepressant doses.
